NDIS Planning with your Local Area Coordinator (LAC)

LACs are here to help you at every stage

This page contains useful information for people who have already connected with a Baptcare Local Area Coordinator (LAC).

If you’re living with disability, or caring for someone living with a disability, Baptcare LACs can support you at every stage of your journey. If you’re just starting your NDIS journey or would like to take your first steps to getting a Baptcare LAC click here.

Once you know you can access the NDIS, your LAC will set up a planning meeting with you. This is where you tell your LAC about the goals you have and what you’d like to achieve. Everyone’s goals are different – there’s no right or wrong. Think about what’s important to you, and things that might make your life more fulfilling or enjoyable. Perhaps you’d like to develop skills to become more independent; enrol in education; get or keep a job; make friendships in your community.

Your LAC will work with you to see how best to support you to achieve your goals. Usually bigger goals are broken down into small, short-term goals that build one by one, leading towards your long-term goal.

For example: If your long-term goal is to move out of home to live with a friend, your first short-term goal may be to learn how to prepare meals. Your second short-term goal may be to learn about budgeting.

Your Baptcare LAC will work with you to create a plan to achieve your goals. Then, your LAC will send this plan back to the NDIA for approval. You can view information about creating your plan here.

Your LAC will talk to you about the steps you need to take next.

The NDIA must approve your NDIS plan initially, then once it is approved, your LAC will give it to you in person or mail a paper copy out to your preferred address.

Once your NDIS plan has been approved your Baptcare LAC will help you work out how to use your plan.

Remember, during your planning conversation, you can ask for a copy of your plan in a variety of accessible formats. These include Braille, electronic text (on CD), large print or audio (on CD), or you can have it translated into your preferred language. Once your NDIS funding is approved, you are ready to start using your plan.

Your LAC will connect you with services and support in your area. These people and organisations are there to help you achieve the goals set out in your plan, and help you connect with your local community.

Your LAC will also help you get the best possible value out of the funding you’ve been allocated by the NDIS.

Once you start using the services and supports set up by your LAC, they’ll check in with you to see how things are going.

You can contact your LAC anytime with questions, concerns or feedback. And don’t forget to let them know if your circumstances change. For example if you move house or your living situation changes.

You can manage your plan at the myplace portal, which you can access via the Australian Government’s myGOV website my.gov.au.
Note, on these websites you may be referred to as an ‘NDIS participant’ or if you’re a carer, ‘a nominee’.

Your Baptcare LAC understands that life is constantly changing. We will work with youth make sure your plan is updated.

About 6 weeks before that 12-month mark, your LAC will talk to you about what is working and what isn’t working.

What will be in my plan?

Information about you – including basic information about your disability, your day-to-day activities, where you live, who you live with, or who cares for you.

Family and friends – includes information about the support you get from family and friends that is not funded but will help you pursue your goals.

Services and community groups – includes information about services and supports funded and delivered by community or other government services like support groups, health centres, libraries and public transport.

Your goals – includes the current goals you would like to pursue as part of your plan and the long-term goals you have identified for your life. Goal-setting is an important part of the NDIS. The supports you receive may help you pursue the goals in your NDIS plan.

Funded supports – tells you what funding you have been allocated in each support category and what this funding is for. You may not have all the support categories funded in your plan. Some people might have one or two support categories funded and others may have more.

The number of funded supports depend on your individual needs and may change from plan to plan based on the supports and services you need. Your funding is based on what is reasonable and necessary for your needs, in addition to the support provided by family, friends and other community and government services that you need to live your life. You must only use your NDIS funding on supports and services that are related to your disability.

An Ordinary Life
The factors of an ordinary life – a good life – come from meeting your own basic needs. The need for friends, family and community as well as for achievement, mastery and recognition – in order to fulfil your potential.

This includes positive relationships, a sense of belonging, individual autonomy, active involvement in decision-making, active engagement in community, using your unique strengths in ways that provide a challenge, and making a contribution.

Community capacity building
Capacity Building Supports are designed to help build skills and independence in your daily life, and to help you pursue your goals.

The National Disability Insurance Agency (NDIA) will assess your needs, and your short and long-term goals during a planning meeting to help identify which Capacity Building Supports might need to be funded. It’s important to know, that you will not necessarily be funded for all supports.

Who is Baptcare?

Baptcare draws on more than 75 years of experience in aged care. We have delivered government-funded Aged Care Home Care packages and services for more than 20 years. We became an NDIS partner in Tasmania in 2013  and in South Australia in 2018.
