Three steps to get ready for a move to Baptcare Coasthaven Community

Our new residential aged care home is almost complete, ready to welcome residents in early 2019.

If you or someone you care for is interested in applying for and securing a place at Coasthaven we recommend that you start completing the following steps now, as this process can take some time.

If you need help understanding this process and applying for government funding, call 13 BAPTCARE (13 22 78) and our friendly team will guide you through the process.

1. Get an aged care assessment
To find out if you are eligible to move into an aged care home you’ll need to meet with a member of the Aged Care Assessment Service (ACAS) for an assessment. An ACAS team member will arrange a time to visit you to discuss how you are managing day-to-day and your care needs. To organise an ACAS assessment visit MYAGEDCARE or call My Aged Care on 1800 200 422. Or, call 13 BAPTCARE (13 22 78) and our friendly team can help you to organise this.

We will ask for your ACAS assessment referral code number and/or a copy of your support plan as part of your application for a place at Coasthaven.

2. Get an income and asset assessment test
To determine the amount and type of Government funding available for your care, you’ll need to complete an income and asset assessment test. For more information about aged care funding and costs, visit myagedcare.gov.au or apply for the test at humanservice.gov.au.

Baptcare Coasthaven Community is a concessional aged care site, meaning there are places available for DHHS Public and Community Housing clients and other government supported clients.

3. Do you need an authorised representative to act on your behalf?
If you need help managing your affairs you can choose to give someone you know and trust the power to make decisions for you, also known as a Power of Attorney. This will allow the person to manage your affairs when you do not want to, or are no longer able to. To organise a representative, call My Aged Care on 1800 200 422 or complete the Appointment of a representative form.

Once you have completed these steps, you can let our friendly team know and we will post you out a Baptcare application form.
