Horizons is a Tasmanian short-term outreach program, for severe mental health support

The Horizons team provides short-term, outreach support assisting people in building strategies, skills and resources to work towards individual goals for their personal recovery. Individual, client-led support can be provided for up to 26 weeks.

Horizons is supported by Primary Health Tasmania under the Australian Government’s Primary Health Networks Program and is focused on each participant’s unique mental health experience, responding to their needs and strengths. The program equips participants with skills to manage their mental health issues – even when severe – to lead fulfilling lives.

Who is Horizons for?

People who experience episodes of severe mental ill-health – no formal diagnosis required. The program supports participants to identify their needs and strengths – empowering them to set goals and supporting them to find and use the tools to achieve them. What is involved in the program?

Our team are here to help participants create a pathway to mental wellbeing, offering them psychosocial support for up to 26 weeks. The program involves one-on-one sessions with one of our team, as well as access to a range of group sessions.

How long is the program?

The evidence informed short-term program offers support for up to 26 weeks

Does this program need a referral?

Yes. In most areas, Horizons is by referral only. GPs, community service organisations, clinical services, allied health professionals, psychiatrists or psychologists can refer people.

Self-referrals can be accepted for certain rural areas which includes, King Island, Flinders Island, Break O’Day, Dorset, Glamorgan-Spring Bay, West Coast, and Circular Head.

Mindset Tasmania delivers a range of psychosocial programs across Tasmania that build people’s capacity to improve their mental health head-on and make progress towards their recovery. Mental Health Practitioners and Peer Workers use evidence-informed practices and work with clients collaboratively to identify goals and remove barriers to living the life they want to lead.
