Extended Aged Care at Home Dementia (EACH D) Packages

Extended Aged Care at Home Dementia (EACH D) packages are individually planned and coordinated packages of care tailored to help older Australians living with Dementia to remain living in their home.

EACH D packages are flexible; services and supports are tailored to the individual clients' needs, which are assessed in conjunction with the client and their family.

Appropriate services and supports may include:

  • care management and coordination
  • technical nursing care
  • care by an allied health professional such as a physiotherapist, podiatrist, diversion therapist, occupational therapist, continence specialist or other type of allied health care
  • personal care
  • lifestyle options
  • home help
  • gardening
  • meaningful Dementia-specific activities
  • carer support, in-home respite, assistance to access residential respite
  • continence management
  • mobilisation aids

It is important to know that the services Baptcare provides can change, as care needs change.

An individual may be offered an Extended Aged Care at Home Dementia package if there are packages available in the area where the frail older person lives with Dementia and the person has:

  • been assessed by the Aged Care Assessment Services (ACAS) as needing high level care and has Dementia with associated behaviours of concern;
  • expressed a preference to receive an EACH D package, having been advised of alternative options.
  • been assessed by the Care Manager to ensure that care needs are able to be met within the program guidelines.
  • been assessed against the EACH D eligibility criteria.

ACAS assessments are free of charge. Find the Aged Care Assessment Services Team nearest to you.

Care recipients may be asked to pay a fee for an EACH D package of care. The amount charged forms part of an agreement between the recipient and the service provider and follows guidelines set by the Commonwealth Government.

Referrals can be made to Baptcare’s EACH D program by the Aged Care Assessment Services, GPs, Hospital liaison staff, Post acute care, councils, a relative or the person needing care. The referral can be made by phone, fax or post.
