Baptcare’s Community Aged Care division delivers innovative, flexible case management services and programs that enable clients to remain living in their own homes and stay connected to their local communities.

An exciting leadership opportunity is available to join the 2009/2010 Aged Care Employer of the Year with the Western Metropolitan Community Packages team based at Sunshine.

The Senior Care Manager position is an opportunity to for you to demonstrate your knowledge of community aged care by providing advice and support to a team of dedicated health professionals.

To be successful in this role you will require:

• · Highly developed assessment, care planning, monitoring and evaluation skills
• · Demonstrated leadership and team building ability
• · A commitment to innovative thinking to assist our clients
• · Excellent written and oral communication skills
• · An understanding of and experience in the community care sector
• · A desire to achieve winning results for clients, families and communities

Postgraduate qualifications would be highly regarded. Excellent problem solving skills, exceptional customer service skills and the ability to work both autonomously and within a supportive team are essential.
