Transitional care packages

Transitional Care Packages (TCP) are home based rehabilitation programs aimed at supporting and enhancing your return to good health after a stay in hospital.

Your Care Manager will work with you to identify your goals and arrange appropriate support which may include physiotherapy, occupational therapy, nursing and personal care.

Your Care Manager will be the contact point for you and your family. Your Care Manager will liaise with other service providers on your behalf to make things as easy as possible for you.

This service is only available in Devonport, Tasmania.

Services and support that may be provided through a TCP package:

All support provided will be tailored to your needs based on your assessment and may include:

  • Case management and coordination
  • Personal care
  • Technical nursing
  • Continence management
  • Physiotherapy
  • Podiatry
  • Occupational Therapy
  • Nursing
  • Other rehabilitation services as required
  • Day activities
  • Transport.

Eligibility

To be eligible for a TCP package you must:

  • Have been assessed by the Aged Care Assessment Team (ACAT) as requiring transitional care at home.
  • Be assessed by the Care Manager to ensure that their care needs are able to be met within the program guidelines.

Admission to a TCP package will be decided on the basis of need for care as assessed against the TCP eligibility criteria.

Baptcare caters to the care needs of all individuals regardless of ethnic background, religion or income.

Fees and charges

There is a fee that clients are asked to contribute towards their weekly package of care. These are in keeping with the Department of Health and Ageing guidelines. If you receive a full pension the fee will not exceed 17.5% of your income.

These costs will be discussed with you at the time of admission to the program. Clients will not be disadvantaged from accessing the program if they experience financial difficulties.

Program duration

The TCP offers a maximum of 84 days (12 weeks) of rehabilitation services at home. We aim to achieve your goals within this time with the support of your Care Manager and visiting allied health practitioners. Extensions are also available for special circumstances.
