Community Nursing Service (CNS)
CNS provides a variety of services to residents of the City of Moreland delivered by a team of nurses, personal carers and podiatrists. Nursing and personal care services are delivered within your home. Podiatry can be delivered either in your home or in the clinic at CNS in Brunswick.
Nursing and personal care staff can also provide services to eligible Department of Veterans Affairs people in the City of Moreland.
CNS can also be the gateway for residents of the City of Moreland to access other Home and Community Care (HACC) Services from other HACC providers.
You can access services through a Home And Community Care (HACC) Assessment. CNS is a designated HACC Assessment Service within the City of Moreland. If you live in the Moreland area, all you will need is a referral by a doctor, hospital, informal carer or even yourself. An assessment visit generally occurs within a week of referral.
